Practice & Policies

All office visits must be scheduled in advance. Please call the office to schedule an appointment. Both inpatient and outpatient surgical procedures are performed at Memorial Hospital of Union County. Should you need to cancel an appointment, 24-hour notice is required so we might offer that time to another patient.

• Picture identification
• Completed patient registration forms
